Building A Stronger Health Care System

Family Health Teams are the cornerstone of the provincial government’s plan to build an improved health care system. Here at the CBFHT, we wholly subscribe to this ethos.

CBFHT helps to promote a stronger health care system by:
  • Ensuring easy access to physicians and other healthcare providers
  • Removing pressure from the hospitals and the healthcare budget by ensuring problems are diagnosed early and treated appropriately
  • Reducing the strain on hospital emergency departments by offering evening and weekend clinics
CBFHT helps to promote individual health by:
  • Providing accurate, up-to-date information about the best ways to prevent disease
  • Helping individuals understand the risk factors for diseases, and provide appropriate screening and follow-up
  • Offering strategies to help manage chronic illnesses like Diabetes, asthma, and COPD
  • Implementing state-of-the-art technology to improve records and information management so that health information is securely shared with other health care professionals
  • Working with patients and families to help navigate the health care system so that care is better coordinated

Central Brampton FHT Recognized for Excellence in Diabetes Care

In 2017 the Central Brampton Family Health Team was selected as one of the recipients of Diabetes Canada’s prestigious Standards Recognition Program (SRP).

The Diabetes Standards Recognition Program recognizes Canadian diabetes education programs that have demonstrated a commitment to providing:

  1. High quality self-management education to patients that is supported by an inter-professional health-care team with specialized knowledge of diabetes
  2. Providing patients with the best practice care as dictated by Diabetes Canada’s Clinical Practice Guidelines

Receipt of the SRP requires that a diabetes education program has met 80% or more of the performance measures and criteria, as evaluated through a rigorous peer-reviewed process.

Our diabetes education team, consisting of our physicians, Pharmacist, Dietitian, Health Educator/Kinesiologist, Nurses, NPs, and administrative staff, is committed to providing patients and caregivers with compassionate, timely, and evidence-based care each time. This is demonstrated by continuous positive feedback received by our patients.
