The Central Brampton Family Health Team is an interprofessional primary care team that comprises of over 10 administrative staff members, 9 family physicians, 6 nurses, 3 social workers, 2 pharmacists, 2 registered dieticians, a health educator, a lactation consultant, a patient navigator, and a respiratory therapist. The goal of our team is to provide the Central Brampton community with all the resources necessary to support their health at every stage.
The Central Brampton Family Health Team has been serving the community for over 20 years.

To create and deliver a team-based model of lifelong care that optimizes health for the individuals, families and communities we serve, through a patient-centred collaborative and comprehensive approach to family health care.
Unfortunately, we are not accepting new patients at this time.
Our family health team offers care in the areas of primary care, family medicine, maternal and prenatal health, pediatric care, mental health services, lung health, healthy lifestyle planning (including diet, exercise and weight management), diabetes management, social service navigation support & senior health and wellness services.
Our primary care services are limited to patients who are registered under our family physicians.
Patients who are registered under the following family physician offices can be referred to services under our allied health team: Gillingham/Central Brampton Family Health Organization, BramEast Family Health Organization, Kennedy Gateway Family Health Team or the Northwest Brampton Family Health Organization.
