PATIENT INFORMATION:

  • Health Card & Personal Contact Information
    As per Ministry of Health requirements, patients must bring and present a valid OHIP card for ALL medical visits.  The card must contain accurate personal information. When you change any personal information please contact Service Ontario (http://www.ontario.ca/welcome-serviceontario), hospitals, labs and/or other doctors’ offices to update your records.
  • The government will not cover medical services for anyone without a valid health card, therefore, a fee for the appointment will be charged if you have an expired/invalid health card. A fee for a medical visit for Quebec and non-OHIP patients must be paid at the time of service.
  • In accordance with the College of Physicians, your patient profile which includes your personal contact information (home address, work/home phone, etc.) including your emergency contact must be maintained and updated when changes occur.  Consequently, upon arriving for your appointment, we will review your demographics with you to ensure your information is up-to-date and accurate.
  • IMPORTANT NOTICE THE PROVINCE OF ONTARIO HAS SET AN OFFICIAL END DATE FOR RED AND WHITE HEALTH CARDS!AS OF JULY 1, 2020, THESE CARDS WILL NO LONGER BE ACCEPTED FOR APPOINTMENTS.IF YOU PRESENT WITH A RED AND WHITE HEALTH CARD AFTER JULY 1, 2020, THERE WILL BE A CHARGE FOR YOUR APPOINTMENT.PHOTO HEALTH CARDS CAN BE OBTAINED FOR FREE AT SERVICE ONTARIO LOCATIONS.

Refill of Prescriptions
If you take regular medications it is best to make an appointment with your physician when you get to your last refill so you do not run out before you can be seen. We do NOT refill medications over the telephone. Additional prescription refills requested by fax or phone by a Pharmacist will NO longer be authorized. Patients will need to book appointment with their physician for refills. Your pharmacy may be able to ensure you don’t run out of essential prescriptions while waiting to get an appointment.

Confidentiality
Confidentiality is of the utmost importance and will be strictly maintained. Your medical information will NOT be released to anyone (including members of your family) without your written permission. If we are unable to reach you directly over the telephone we will leave a message for you to contact us.

Patients are permitted to restrict the disclosure of such information. For this reason and in conjunction with the College of Physicians and Surgeons of Ontario regulations, we are required to obtain written express consent from the patient before we disclose the patient’s personal health information. It is permissible for a parent or legal guardian to manage these tasks for a minor and by default a parent or guardian is assumed to have authorization for a minor. Children sixteen (16) years of age or older MUST also grant authorization to a parent or guardian.

Authorization to Disclose Forms can be requested at reception desk, from your family physician or health care provider at the office.

After Hours Clinic Information

We provide after- hours walk-in clinic times on Monday, Tuesday, Wednesday and Thursday evenings from 4:00 pm to 8:00 pm. Patients are seen in the order in which they arrive. Registration ends at 7:00 pm. These clinics are offered every day except Sunday and Friday evenings so that patients can have their health care needs addressed as comprehensively as possible in the medical clinic where their physician practices. We advise and encourage patients to utilize the after-hours clinic at CBFHT for their urgent care needs rather than other, outside walk-in clinics. This will ensure continuity of each patient’s health care as your family physician does not receive any consult information from other walk in clinics you have visited. In addition to our evening walk-in clinics, there is a doctor on call every day to see patients who cannot be scheduled in with their regular doctor that day. You can call the clinic to book an appointment for same-day appointments. If you need more information about our after-hours clinics please call us at 905-451-1937. Telephone health Advisory Service (THAS) is available to all rostered patients at 1-866- 553-7205.

Please note that a call to Telehealth Ontario does not replace a 911 call. A call to 911 is always the first option in emergency situations.

Only requisitions that have been received from one of our clinic physicians will be fulfilled. Requisitions from specialists or physicians not affiliated with the clinic will not be processed.

What Is Telehealth Ontario?

Telehealth Ontario is a free, confidential telephone service an individual can use to receive health advice or general health information from a Registered Nurse. This call means quick, easy access to a qualified health professional, who can assess symptoms and determine a plan of action.

How Does Telehealth Ontario Work?

When an individual calls Telehealth Ontario, they talk directly with a Registered Nurse. They are asked to describe symptoms and answer questions that will allow the RN to best assess the seriousness of the problem. Based on the assessment, the RN can advise proper self-care, recommend a visit to a health practitioner, or provide phone numbers of nearby community resources. The Telehealth Ontario service is provided in English and French with translation support for other languages and a direct TTY number for those with hearing and speech difficulties. Callers can also be connected to medication or health information audio tapes.

When Should An Individual Call?

When Should an individual Call? When they have a general health question but are not sure of the exact nature of its severity. Telehealth Ontario answers questions and provides information that ensures proper care is received, all while following confidentiality laws. Health-related concerns that Telehealth can provide advice about include: Symptoms that could require medical attention - Illness or injury - Chronic illnesses - Nutrition and healthy lifestyles - Teen health and lifestyle issues
