Position Summary:

Reporting to the Executive Director of Central Brampton FHT, the Quality Improvement Decision Support Specialist is responsible for supporting multiple family health teams in their quality improvement planning, decision making and implementation activities. This will include supporting teams in the implementation of data quality initiatives, improving the flow and use of information, developing queries and analytical products that support boards and leaders in their quality improvement goals and teams engaged in clinical process change.  The incumbent will also be expected to participate in broader data harmonization initiatives and champion the use of data and performance analytics within and across Ontario’s family health teams.

Job Responsibilities:

  • Work with a steering committee and stakeholders to build decision support capacity
  • Be a subject matter expert in data integrity and lead the analysis of data quality issues, using problem-solving methodologies to recommend corrective and preventative action
  • Collaborate closely with multi-disciplinary teams engaged in quality improvement helping to identify performance measurement information and data quality needs
  • Design, test and deploy tools and processes for extracting and managing performance data from EMR to dashboards
  • Identify gaps in data availability, and define and implement solutions to close the gaps
  • Support the work of boards and quality improvement committees in the development and implementation of quality improvement plans for all partners.
  • Provide consultative and analytical support regarding the collection, interpretation, analysis and presentation of data to help teams ensure that clinical information management practices support best practice standards, such as Patient satisfaction surveys.
  • Act as a change agent and coach to inter-disciplinary teams on data management and performance measurement, such as improving access and integration for patients
  • Create effective working relationships with multiple teams and manage data quality, business process improvement, report development and other projects using effective project management practices
  • Work with your peers across the province on standards development and data harmonization priorities
  • Conduct analysis and develop reports and presentations using statistical reporting packages, and present this information to a variety of audiences
  • Monitor and evaluate clinical information system functionality from a user and decision support perspective

Key Qualifications:

  • A university degree in a related discipline (Business Administration, Statistics, Health Sciences or Information Management) or equivalent education and experience.
  • Demonstrated expertise in data quality and clinical information management best practices
  • Currency in applicable privacy and confidentiality policies and laws regarding the collection and use of health information is required.
  • Knowledge of primary care and demonstrated interest in the use of data for quality improvement
  • Strong applied knowledge of Oscar EMR systems and proficiency in programming languages and reporting writing tools (SQL, SSRS, SSIS, SSAS or Crystal Reports <as applicable>) will be an asset.as will a demonstrated capacity to learn similar packages quickly
  • Sound knowledge of performance improvement techniques and practices used in healthcare settings, process redesign and system implementation experience
  • Highly effective communication, presentation and interpersonal skills and a proven ability to develop and maintain strong relationships with partners and community of practice groups.
  • Demonstrated proficiency in quantitative analysis and report writing skills
  • Strong report writing skills using tools such as NPR, Cognos, SQL, SAS, Access and Crystal Reports, Excel,

PPT and outlook.

  • Ability to work remotely with partners, including travel to support other sites
  • Experience with software such as Zoom, Skype and Team viewer
  • Ability to handle a variety of concurrent assignments and to work independently
  • Valid Ontario driver’s license

Nurse Practitioner (1.0 FTE) – 2130 North Park Drive Location

Nurse Practitioner-PHC 1.0 FTE

The Central Brampton Family Health Team (CBFHT) is expanding to create inter-professional primary care (IPC) team by improving health care capacity through a team-based accessible approach to care. IPC overall goal is to target specific health needs within the community served through the implementation of programs with a focus on mental health, senior wellness, diabetes, and other programs.

CBFHT is currently recruiting a dedicated Nurse Practitioner that has a keen interest in chronic disease management (hypertension, lung health), a strong background in geriatric medicine/senior’s wellness, and the ability to work in collaboration with Central Brampton FHT full Interprofessional Healthcare Provider (IHP) Team.

The NP will participate in the development, implementation, monitoring, and evaluation of health prevention programs at CBFHT. The Nurse practitioner will share their expertise and provide a range of independent and collaborative advanced nursing care and knowledge of health promotion, disease prevention, senior wellness, and chronic disease management with referring primary care providers and their patients.

Experience in current Chronic Disease Management and Geriatric Wellness are preferred.
Main Responsibilities:

  • Perform individual assessment, diagnosis, treatment, health education/counseling, and other clinical activities according to CNO standards of practice for RN (EC)
  • Provide care to patients referred to CBFHT following guidelines for health promotion, disease prevention, chronic disease management, and geriatric care
  • Consult as appropriate, with members of an interdisciplinary team, referring primary care provider, the local hospital, community care partners, specialists, and the surrounding community to assess, plan, implement, evaluate and coordinate a patient’s plan of care and treatment
  • Ability to provide support to families and patients through the utilization of community partners and resources
  • Understand the importance of building relationships with community partners such as First Link, Home, and Community Care and local geriatricians
  • Ensures continuity of care by maintaining accurate up-to-date clinical records, available to consult with a primary care provider, monitoring patients on a regular basis, and case-conferences with members of the CBFHT team as required
  • Increase access to enhanced assessment, diagnosis, and treatment within the community and/or target population
  • Advocate, educate, lead, provide mentorship, and consult as needed
  • Actively participates in staff and team meetings as appropriate to facilitate program development and implementation

Diagnostic Skills and Knowledge

  • Experience in administering and interpreting cognitive tests such as MoCA, RUDAS, Trails A, Trails B
  • Ability to perform in-depth holistic patient/family assessments, plan, implement and evaluate care for complex care situations, and guide interdisciplinary team members in care planning.
  • Ability to synthesize a comprehensive geriatric consult note reflective of the completed geriatric and cognitive assessments
  • Monitor the ongoing therapy/intervention of patients with Dementia through scheduled follow-up appointments
  • Knowledge of Advanced Care Planning, Advanced Care Planning training an asset
  • Able to communicate with patients, their families, and other relevant individuals in order to follow through with the care plan
  • Provide clinical advice and support for families living with or caring for a person with dementia
  • Experience in managing and monitoring patients with hypertension
  • Experience in managing and monitoring patients with respiratory conditions

Qualifications:

  • Current extended class certificate and registration with the College of Nurses of Ontario as a Nurse Practitioner-Primary Health Care (NP-PHC)
  • Graduate of a recognized Primary Health Care Nurse Practitioner program
  • Candidates require a minimum of 2-3 years of clinical experience as a Nurse Practitioner, preferably within a multidisciplinary primary care setting

  • Strong clinical and effective time management skills
  • Demonstrated ability to work independently and collaboratively as a member of an interdisciplinary team

Health Educator (1.0 FTE) – 2130 North Park Drive Location

Health Educator – 1.0 FTE

The Central Brampton Family Health Team (CBFHT) provides primary health care, including chronic disease management and health promotion and disease prevention, to a rostered population of patients in Brampton and surrounding communities.  The FHT utilizes an interdisciplinary team of health care professionals who work in a collaborative model of care.

The Health Educator (HE) plays an integral role in the care of the patients of the Central Brampton Family Health Team. The role of the HE in the Central Brampton FHT requires a thorough knowledge of the development and implementation of patient-focused health education programs which may be delivered in either one-one, a community-based workshop setting or both.   A solid understanding of adult education principles and the capacity to adapt programs based on patient demographics and need underpin success in this role.  Effective presentation skills and the ability to build programs from the bottom up are key skills in this role. The role also requires a high level of interpersonal skills to deal with patients and their families in a holistic and compassionate manner.

The Health Educator works closely with other members of the Family Health Team to ensure that the patients we support are aware of the importance of a healthy lifestyles, nutrition, exercise, risk avoidance, health screening and immunization to achieve optimal health.

Responsibilities:

Assessment and Treatment

  • Work collaboratively with Program Coordinator and program team members to manage the needs of patients and to ensure that patients are screened for their level of understanding about their disease or health condition and the benefits of education.
  • Determine health promotion priorities and develop comprehensive, patient centered care plans through clinical assessment.
  • Provide patient education and counselling on a variety of lifestyle topics
  • Experience performing orthopedic assessments for the purpose of developing customized therapeutic exercise programs (both rehabilitation and general fitness programs) for a variety of conditions
  • Play an active role in the CBFHT Diabetes Management Program, Geriatric Program, Metabolic Syndrome Program, and Lung Health Program.
  • Through collaboration with the allied health care team, develop and implement comprehensive patient education programs focused on specific health topics including diabetes, lung health, hypertension, geriatric health, and osteoporosis
  • Identify educational resources including, but not limited to, internet-based tools, brochures, videos and lectures to support patient self-education.
  • Ensure that all FHT members are aware of the educational resources that are available to patients/clients.
  • Develop follow-up programs and services to ensure that patients are provided with educational materials.
  • Help facilitate the incorporation of proven strategies for individual family physician practices to provide more effective care delivery.

Organizational

  • Participate as a team member in the development and delivery of targeted programs including but not limited to chronic disease, health prevention, child and women’s health.
  • Collect data as required for Quality Improvement and Program target performance measures.
  • Provide timely and informative reports as directed and requested by the Executive Director.
  • Participate in team building, program development and improvement activities.
  • Participate in staff meetings, to exchange ideas, receive mutual support and monitor program outcomes as well as present case studies.
  • Assist in the development and implementation of the clinical model and guidelines for collaborative care as required.
  • Comply with established personnel policies, procedures, standards and guidelines for operation.
  • Identify gaps and needed modifications in services.
  • Other duties as assigned.

Qualifications and Requirements

  • Graduate of an accredited school of Occupational Therapy and/or Kinesiologist (R Kin)
  • Current registration with the College of Occupational Therapists of Ontario and/or Kinesiologist
  • Membership with professional association
  • Additional university or community college programs specializing in community education, health promotion, adult education, marketing, communications or community engagement an asset
  • Practical experience in the development of health education programs
  • A solid understanding of chronic disease etiology and treatment options
  • Orthopedic assessment skills and exercise program development for chronic disease conditions and musculoskeletal injuries
  • Demonstrated skills in program planning, implementation and evaluation
  • Evidence of experience in individual and group presentation skills
  • Knowledge of community resources connected to the provision of health promotion and illness prevention related services
  • Knowledge of client-centered philosophy, change management, basic principles and practices of community service delivery and chronic disease management models
  • Ability to work effectively, both independently and as part of an inter-disciplinary team
  • Flexibility and ability to adapt to change
