Covid@Home Monitoring Program
- Are you a patient of Central Brampton Family Health Team at 60 Gillingham Drive?
- Have you been recently diagnosed with Covid-19?
- Are you 18 years of age or older?
If you answered “Yes” to the above questions, speak to your family doctor and get referred to the Covid@Home Monitoring Program. You will receive regular telephone check-ins by a Registered Nurse and a free oximeter to test your oxygen saturation levels. The program helps patients with a positive diagnosis to measure and report their vital signs and symptoms from the comfort of their home. To view or download the flyer, please
