Purpose
Our Seniors Wellness Program is a health promotion and preventative program designed to increase access to care and assist adults 60 and older to lead their best quality of life, as independently as possible.
Focus
This program offers comprehensive assessment by a multidisciplinary team to support older patients and families (when necessary) with the navigation of medically, emotionally, and socially challenging situations that prevent or hinder them from achieving their health goals.
Goal
To provide patient-focused, preventative services in a timely manner that promotes, teaches, and supports both patients and their families in maintaining and/or achieving their health-related goals.
Program Activities
The Central Brampton Family Health Team is proud to support the senior members in our community with a tailored cognitive health program through our MINT Memory Clinic.
What is a MINT Memory Clinic?
MINT stands for Multi-specialty INterprofessional Team.
In MINT Memory Clinics, each health care professional has received standardized accredited training to deliver high-quality dementia care.
From the first referral to follow-up, MINT Memory Clinic teams work within a shared care approach with the patient’s family physician. Patients with memory concerns are referred by their family physician to a MINT Memory Clinic for comprehensive assessment and care planning. The MINT Memory Clinic provides a detailed and proactive care plan to the family physician, and refers to specialists when necessary.
What makes our program unique?
Ours is the first NP-led clinic of its kind! Our Nurse Practitioner (NP) alongside our patient navigation nurse, pharmacist and admin team work closely with your family doctor, community agencies and specialist physicians to provide compassionate, person-centred care for you and your family members. Our Family Health Team model of care also means that patients seen may also be referred to members of our team, including our Kinesiologist, Dietitian, Respiratory Therapist, and Mental Health Counsellor where applicable.
How can you access this clinic?
This memory clinic option is available to patients who are referred to us by their family doctor. This applies to clinics to which the Central Brampton Family Health Team is currently partnered.
