Understanding the Primary Care Program

Purpose 

The Primary Care Program at Central Brampton Family Health Team (CBFHT) aims to promote overall wellness along with providing patient with knowledge and skills to manage wellness and chronic disease processes. Through comprehensive education, personalized guidance, and access to specialized healthcare professionals, the program supports individuals in improving their health outcomes and quality of life.

Goal 

To provide patient-focused, chronic disease preventative services in a timely manner that promotes, teaches, and supports both patients and their families in maintaining and/or achieving their health-related goals.

Program Activities 

    • Diagnosis and treatment of common illness and injuries
    • Chronic disease support and management including hypertension, INR
    • Diabetes care and management:
      • Our Diabetes Management Program is designed to provide patients with a comprehensive and interdisciplinary approach to diabetes care. We understand that diabetes is a complex chronic illness and providing care requires consideration of all social and health components. We offer a team-based approach to diabetes management where patients have access to members of our interdisciplinary team.
    • Post hospital discharge:
      • Our Registered Nurse conducts a comprehensive assessment for rostered patients following a hospital admission. Along with the nursing assessment, a medication review is completed by our Clinical Pharmacist. This process ensures continuity in patient care between primary care and other healthcare providers. 
    • Clinical Pharmacy services:
      • Our pharmacists are the medication experts of the team. They offer evidence-based advice on how you can benefit the most from medications and how to minimize drug interactions and side effects. They can help personalize your medications to your specific conditions, needs and preferences. Our pharmacists are actively involved in other programs including the Diabetes Education Program, MINT Memory Clinic and Smoking/Vaping Cessation Program.
