Purpose 

The Health & Lifestyle Education Program at Central Brampton Family Health Team (CBFHT) aims to promote overall wellness and empower patients to adopt healthy lifestyle choices. Through comprehensive education, personalized guidance, and access to specialized healthcare professionals, the program supports individuals in improving their health outcomes and quality of life. 

 

Eligibility 

All rostered patients of CBFHT are eligible to participate in the Health & Lifestyle Education Program. Whether individuals are managing chronic conditions, seeking preventive care, or aiming to enhance their well-being, the program offers inclusive support tailored to diverse healthcare needs. 

In addition, community members who are not rostered to CBFHT may also participate in webinars and workshops offered through the program. 

 

Goal 

The program’s main goal is to educate and empower participants to make informed decisions about their health and lifestyle. By collaborating with our registered dietitian, health educator who is a (registered kinesiologist) and leveraging resources from the diabetes management program, the program aims to foster sustainable behavior changes that lead to improved overall health outcomes. 

 

Program Activities: 

 

  • Nutritional Counselling to prevent or delay the progression of different chronic diseases which 

includes diabetes, high cholesterol, hypertension and liver diseases. This nutritional counselling, led by our registered dietitian provides personalized nutrition counseling sessions to empower participants to develop healthy eating habits, learn about their individual dietary requirements, and how to manage weight effectively 

  • Physical Activity Programs: Guided by our health educator (registered kinesiologist) provides tailored exercise programs designed to enhance fitness levels, improve mobility, and promote overall physical well-being. 
  • Health Workshops and Seminars: Educational sessions cover various topics including Chair Exercise, Highway to Health, Starting Solids, etc.

Highway to Health: Lifestyle Workshop (8 Weeks of Group Session Online)

Are you struggling to lose weight? Are you frustrated with your diet? Finding it difficult to incorporate movement into your lifestyle? Highway to Health is the solution for you!

Join us every week to talk about making healthier choices to shape a healthy lifestyle. This 8-week workshop is led by our Registered Dietitian and our Certified Diabetes Educator/Registered Kinesiologist

It can be hard to know where to start with high cholesterol. Our team offers a compassionate approach using tools such as moderation, mindful eating, consistent pressure monitoring, and Mediterranean dietary habits that you can use to improve your health outcomes.
