Our Current Programs

We provide a variety of programs and services that focus on supporting and improving the health of the community we serve. Our educational, interactive programs are designed to address our patient needs beyond their primary care visits. Patients can register online to attend workshops and events for our Mental Health, Diabetes Management, Highway to health and Infant Feeding programs.

Diabetes Management

The program works to provide self-management education and support to patients living with diabetes in order to improve overall health and quality of life.

Hypertension Management Program

If you have high blood pressure that has been newly diagnosed or challenging to control, this program may be just right for you. High blood pressure is often referred to as a ‘silent killer’, because individuals often feel fine despite their blood pressure being elevated. Achieving normal blood pressure is important to reduce the risk of heart attack, stroke and other cardiovascular diseases.

Lung Health Screening Program

Chronic Obstructive Pulmonary Disease (COPD) is the third leading cause of death in the world. Smoking is a major contributor to COPD: approximately 20% of smokers develop the disease. Other risk factors such as air pollution, respiratory infection, chronic asthma, genetic factors and occupational exposure to pollutants in the workplace also contribute to the burden of lung diseases. Asthma is another chronic disease which makes breathing difficult.

The Health & Lifestyle Education Program at Central Brampton Family Health Team (CBFHT) aims to promote overall wellness and empower patients to adopt healthy lifestyle choices. Through comprehensive education, personalized guidance, and access to specialized healthcare professionals, the program supports individuals in improving their health outcomes and quality of life. 

Healthy Familiies

Our Healthy Families program is a dedicated primary care initiative designed to support parents and their children from pregnancy through every stage of childhood and beyond. Whether you’re expecting your first baby or navigating toddlerhood and school-age milestones, our compassionate team of doctors is here to provide continuous, family-centered care. From prenatal and postnatal support to routine wellness visits as your child grows, we’re committed to building lasting relationships that prioritize your family’s physical, emotional, and nutritional well-being.

Seniors’ Wellness

Healthier moms have healthier babies. Healthy babies are more likely to develop into healthy children who grow to be healthy teenagers and adults. We offer 4 educational workshops designed to provide moms-to-be and new parents with evidence-based information that will help them to optimize their prenatal health and that of their baby during the first year of life. 

These programs are open to both Central Brampton Family Health Team patients and the community.

Hospital Discharge

The discharge program’s main objective is to facilitate a seamless and well-coordinated transition for patients as they move from hospital care to home or other care settings. Central to this effort are thorough discharge assessments, which entail meticulous review of hospital discharge reports and assessments. The goal of the program is to enhance patient outcomes by facilitating a smooth and organized transition post-hospitalization, promoting patient safety, reducing readmission rates, and improving overall continuity of care.

Mental Health

Central Brampton Family Health Team works collaboratively with your family physician to provide mental health services that focus on strengthening coping skills and improving overall functioning. This is achieved through individual counselling, psycho-educational mental health webinars, psycho-therapy groups, and care coordination/patient navigation. Services are geared towards those with mild to moderate mental health concerns (including, anxiety, depression, and stress related to relationships). Services are offered in English, Hindi, Urdu and Punjabi.

We offer a weekly webinar series that is open to both Central Brampton Family Health Team patients and the community.

Patient Navigation

Navigating community services can be overwhelming, but you don’t have to do it alone. Our Patient Navigation Program is here to guide you every step of the way. Our dedicated Patient Navigator works closely with individuals and families to identify the resources that are right for your unique situation. Whether it’s connecting to healthcare services, financial assistance, mental health support, or social programs, we’re here to make the process easier.

Pharmacy

Our pharmacists are the medication experts of the team. They offer evidence-based advice on how you can benefit the most from medications and how to minimize drug interactions and side effects. They can help personalize your medications to your specific conditions, needs and preferences. Our pharmacists are actively involved in other programs including the Diabetes Education Program, MINT Memory Clinic and Smoking/Vaping Cessation Program.

Primary Care

Primary care serves as the cornerstone of healthcare systems, focusing on comprehensive and continuous medical services aimed at promoting and maintaining individual and community health. It emphasizes preventive care, early detection, and management of common health conditions, ensuring timely interventions and fostering long-term patient-provider relationships. By providing accessible, coordinated, and holistic care, primary care aims to address diverse health needs, improve quality of life, and promote health equity, enhancing overall well-being for patients.

Breastfeeding Program
