Our Current Programs

We provide a variety of programs and services that focus on supporting and improving the health of the community we serve. Our educational, interactive programs are designed to address our patient needs beyond their primary care visits. Patients can register online to attend workshops and events for our Mental Health, Diabetes Management, Highway to health and Infant Feeding programs.

Comprehensive Diabetes Care Clinics

The program works to provide self-management education and support to patients living with diabetes in order to improve overall health and quality of life.

Hypertension Management Program

If you have high blood pressure that has been newly diagnosed or challenging to control, this program may be just right for you. High blood pressure is often referred to as a ‘silent killer’, because individuals often feel fine despite their blood pressure being elevated. Achieving normal blood pressure is important to reduce the risk of heart attack, stroke and other cardiovascular diseases.

Lung Health Screening Program

Chronic Obstructive Pulmonary Disease (COPD) is the third leading cause of death in the world. Smoking is a major contributor to COPD: approximately 20% of smokers develop the disease. Other risk factors such as air pollution, respiratory infection, chronic asthma, genetic factors and occupational exposure to pollutants in the workplace also contribute to the burden of lung diseases. Asthma is another chronic disease which makes breathing difficult.

Highway to Health

Highway to Health is a progressive, holistic 10-week lifestyle program to reboot wellness, regardless of age!

  • Learn strategies to improve heart health and blood sugar levels
  • Improve body composition and overall health
  • Develop more endurance and feel more energetic
  • Improve nutritional status & your relationship with food
  • Discover tips to improve your meal planning
  • Discuss techniques to manage difficult eating patterns
  • Acquire greater skill reading food labels
  • Develop skills to help manage daily stress
  • Bust food and exercise myths
 

Mental Health Program

Central Brampton Family Health Team works collaboratively with your family physician to provide mental health services that focus on strengthening coping skills and improving overall functioning. This is achieved through individual counselling, psycho-educational mental health webinars, psycho-therapy groups, and care coordination/patient navigation. Services are geared towards those with mild to moderate mental health concerns (including, anxiety, depression, and stress related to relationships). Services are offered in English, Hindi, Urdu and Punjabi.

We offer a weekly webinar series that is open to both Central Brampton Family Health Team patients and the community.

Lifestyle Counselling

Poor diet, lack of physical activity and smoking are the underlying causes of most major chronic diseases such as cardiovascular disease, diabetes, osteoarthritis, and even many cancers. There is mounting evidence that physical activity is an important contributor to reducing the risk for cognitive decline with age. Our team of registered dietitians, certified diabetes educators and health educators are prepared to equip you with the tools you need to improve your lifestyle and overall health.

Prenatal, Infant Feeding and Breastfeeding Workshops

Healthier moms have healthier babies. Healthy babies are more likely to develop into healthy children who grow to be healthy teenagers and adults. We offer educational workshops and webinars designed to provide moms-to-be and new parents with evidence-based information that will help them to optimize their prenatal health and that of their baby during the first year of life. 

These programs are open to both Central Brampton Family Health Team patients and the community.

Seniors’ Wellness Program

Healthier moms have healthier babies. Healthy babies are more likely to develop into healthy children who grow to be healthy teenagers and adults. We offer 4 educational workshops designed to provide moms-to-be and new parents with evidence-based information that will help them to optimize their prenatal health and that of their baby during the first year of life. 

These programs are open to both Central Brampton Family Health Team patients and the community.
