Understanding the Lung Health Program

Purpose 

Patients referred to the Lung health Management Program can expect to receive a comprehensive action plan and close follow-up care from the lung health team. This involves having discussions relating to pathology of a diagnosis, education on appropriate medication usage, and ensuring the patient has an up-to-date action plan.

Eligible patients referred to the STOP will receive free nicotine replacement therapy (up to 26 weeks) or recommendations for smoking cessation medication starts (i.e. Champix or Zyban) along with support from a smoking cessation counsellor on the team. 

Focus 

To provide support to patients with respiratory related concerns with a timely review of their symptoms, medications and action plan to reduce unnecessary healthcare related visits and increase the patient’s comfort level with respect to self-management skills.

Eligibility

  • All patients with a respiratory diagnosis or symptoms who are on a puffer can be referred for an assessment and assistance in managing their condition. 
  • Current smokers are eligible to enroll in the STOP Program.

Goal

The goal of the program is to provide assistance to patients with respiratory symptoms. This involves providing education on asthma and COPD, with access to resources that will help in symptom self-management.

Program Activities 

  • Asthma and COPD education with a Respiratory Therapist
  • Smoking cessation support
  • Medication review including education on puffers
  • Spirometry testing
